Intake Form
If you're considering working with me the first step is to get in contact with me. From there I will send an intake form to you. This form provides me with essential information to tailor our sessions to your unique needs.
What the Intake Form Includes:
Personal Details: Your name, contact information, and preferred method of communication.
Funding Information: Details about your funding pathway (e.g., NDIS, Medicare referral, Private) to ensure any applicable rebates are processed correctly.
Additional Notes: A section where you can share any thoughts, feelings, or accommodations that would help me assist you better.
The form is designed to be user-friendly, with mostly optional free-text fields, allowing you to provide as much or as little information as you're comfortable with.
Once submitted, I'll review your responses and reach out to schedule our first session. If you have any questions or need assistance with the form, please don't hesitate to contact me.
Face-to-Face Appointments at Northside
For folk that would prefer to meet in person at Northside, I will respectfully ask for your consent to share your contact details with the wonderful administrative team there. With your permission, they will assist in coordinating your appointments and ensuring you're well taken care of.
Please know that this step is entirely optional. Your comfort and privacy are my top priorities, and I will only proceed with sharing your information if you feel comfortable and provide explicit consent.
Our appointments
I approach our work together through the lens of food and eating. This is quite a broad lens, as many aspects of our lives - emotions, experiences, and environment can impact our ability to nourish ourselves. My aim is to understand these connections and support you in navigating them.
Many folk who come to see me have had previous experiences with dietitians. While there is often a structured process in assessments, I want to be transparent that I frequently go off-script. I recognise that eating and nourishing ourselves is deeply complex. Each person brings their own lived experience, and I believe that you are the expert on your own body.
Often, it's not a lack of knowledge about food that leads someone to seek support, but more, a feeling of stuck-ness. My role isn't to provide a checklist or make you feel ashamed for not "eating good enough." I'm here to listen, understand, and walk alongside you with compassion and respect.
In our first appointment, my intention is to hear your story and understand what brought you here. I appreciate whatever you feel safe and comfortable sharing, whether that's a little or a lot. There's no right or wrong way to speak about your experiences, and you are welcome just as you are.
Appointments are scheduled for 50 minutes. If you feel that a longer appointment would better support you, please let me know.
I integrate elements of Compassion-Focused Therapy (CFT) and Acceptance and Commitment Therapy (ACT) into my practice. By combining these approaches, I aim to support you in developing a compassionate relationship with yourself and food.
